DETAILED NOTICE FOR INVITING APPLICATION

 

Civil Surgeon Gurugram invites applications for hiring building of 40 Urban Health and wellness centre in various parts of Gurugram city, Gurugram. (List Attached)

 

Eligibility of firms/individuals Documents to be submitted:-

 

1. Minimum three to four rooms with two washrooms (Approx

 

covered area should be 600-800 Sq. feet with waiting area)

 

will be required for setting up of Health and wellness centre (U-HWC)

 

2. Area must be on ground floor with proper ventilation. 3. The applicant must have PAN Card must be submitted.

 

4. Quotation rates should be quoted as per square feet.

 

5. Individual owner/RWA/partners are eligible for this application.

 

6. Property should not be disputed in any terms (Like police case,

 

court case, family dispute etc.) 7. If there are more than one application lowest rates will be considered.

 

8. Interested applicants will have to deposit the photocopies of

 

relevant documents for ownership of the property.

 

9. All expenses like water, sewerage and house Tax etc. will be

 

paid by owner.

 

10. Electricity bill will be paid by civil surgeon/ UHWC in-charge and for that sub meter should be fitted separately by owner. 11. Undersigned will have the rights to reject any application

 

without giving any reason. 12. The application is invited for the various parts of Gurugram city

 

(List Attached). 13. Bidder/applicant must submit an affidavit stating that, related property belongs to His /Her /owner/RWA/partners on Rs. 10/-

 

stamp paper.

 

14. Successful applicant shall be awarded with the award of contract for 11 month initially which can be extended further. 15. All applications will be accepted by hand, no application will be

 

considered via E-mail or online. 16. A team from O/o Civil Surgeon would physically visit the location against received applications to check the fulfillment of requirement.

 

Note:-Applications will be accepted dated till 30-12-2022 till 12:00 o' clock and will be opened at 30-12-2022, 3:00 PM same day, in the presence of committee member/members.
